I have read your article on the reporting of AIDS related and defining mailignancies in Africa with alot of interest. I work at the department of Human pathology at the University of Nairobi, Kenya. In collaboration with the Mid-region Aids cancer specimen resource and Prof. Leona Ayers of Ohio state University, we have for the last four years been involved in review and archiving of all malignancies in patients reported to have HIV(project -AIDS CANCER SPECIMEN RESOURCE KENYA). We have in our records material from 2000 to date and have published one poster that was presented at the recent IAP conference in Capetown South Africa. Visit our website.

This visit was most valuable and led to greater rapport between INCTR and ORCI

Please feel free to discuss about the main questions raised at the workshop led by INCTR on Pediatric Oncology at the 3rd International Cancer Control Congress in Italy:

1. What principles underlie optimal pediatric cancer care outcomes and how can they be made practical and implementable in resource constrained environments?
2. How can effective capacity to manage children with cancer be achieved in remote and resource constrained environments?
3. Are ‘levels of care’ appropriate and achievable in resource constrained environments? Discuss.
4. What are the attitudes and practices regarding management of pediatric oncology patients in your country? Discuss challenges and opportunities.

Thank you for your comment Joseph. You are, of course, correct, and this is one of the reasons we are developing this program. The vision is to improve communications with one or more central labs in a country, and to have assistance in consultations, quality control and education from outside. External experts can make up for the lack of qualified specialists at first, although we shall have to develop a "faculty" to undertake this work. We already have more than 20 pathologists williing to help. This will take some organization, which we are working on. In addition, incidentally, the system can be used for consultation on difficult cases - even by institutes unable to upload pathology images, and move towards international multidisciplinary conferences (tumor boards).

Lets communicate

If the telepathology program mentioned aims at providing a framework for review/consultations for sites (including remote locations) by a central lab through the internet, then many of the problems that are due to lack of pathology services in low resource settings, would be effectively addressed.

Joseph.
